Who Owns the AI Agent? Governance Questions Every CISO Must Answer in 2026

The accountability gap no one is talking about.
When an AI agent causes a breach, the CISO still owns it. But most organisations haven’t built the governance structures to prevent it.
This isn’t theoretical. Autonomous AI agents have moved from pilots to production. They’re modifying files, calling APIs, spinning up workflows — executing consequential decisions across banking, healthcare, retail, and supply chains. The governance question is no longer “is our AI safe to use?” It’s “who is accountable when an agent causes harm?”
And most boards haven’t answered that question yet.
The governance gap is worse than you think
In May 2026, OWASP published the first formal taxonomy of risks specific to agentic AI systems. The Top 10 for Agentic Applications formalises threats that barely existed 18 months ago: goal hijacking, tool misuse, memory poisoning, identity abuse, cascading failures, rogue agents.
Microsoft followed with the open-source Agent Governance Toolkit — a runtime policy enforcement system designed to address all ten OWASP risks with sub-millisecond latency. The NSA and allied cybersecurity agencies released joint guidance on securing agentic AI for government and critical infrastructure.
The pattern is unmistakable. The threat taxonomy exists. The enforcement frameworks exist. The regulatory expectations are forming. What’s missing is the organisational accountability layer.
McKinsey’s 2026 research identifies security, risk management, and governance concerns as the most cited barriers to scaling agentic AI. Not technical feasibility. Not cost. Governance.
Deloitte found that 74% of companies plan to deploy agentic AI moderately or extensively within two years. That means the deployment wave is already rolling. CISOs who wait for agents to be in production before building security posture will be playing catch-up in a high-stakes environment.
The questions boards should be asking
Most board-level AI governance conversations still centre on the old questions: data privacy, algorithmic bias, explainability. Those matter. But they’re not sufficient for agentic AI.
When AI systems don’t just suggest actions but execute them — autonomously, at scale, across enterprise workflows — the risk profile changes fundamentally. The blast radius of a compromise becomes enterprise-wide.
Here are the questions boards need to be asking their CISOs right now:
1. Do we have an inventory of every AI agent deployed in production?
You can’t govern what you can’t see. Most organisations don’t have a formal AI agent registry. Shadow AI — unvetted AI tools embedded in vendor ecosystems — has emerged as a distinct and fast-growing sub-category of supply chain risk that existing GRC platforms are poorly equipped to monitor.
A 2026 Panorays study revealed that 85% of CISOs lack full visibility into their third-party threat landscape. 60% of CISOs identify unmanaged vendor AI tools as uniquely risky. Only 22% have formal AI vendor vetting processes.
If you don’t know which agents are running in your environment, you can’t secure them.
2. Do our AI agents have privilege boundaries?
Traditional privilege access management (PAM) frameworks were designed for human identities. AI agents are not human identities. They operate continuously. They cross system boundaries. They execute transactions with consequences.
Do your security architects have an identity and privilege framework specifically for AI agent accounts? Most don’t. And that means you’re extending enterprise privileges to autonomous systems without the guardrails you’d require for any human with equivalent access.
3. What happens when an agent goes rogue?
Not “if.” When.
Do you have incident response playbooks for autonomous system failures? Do your runbooks account for cascading failures — where one compromised agent triggers downstream effects across multiple systems?
OWASP’s taxonomy includes “rogue agents” as a formal threat class. Your incident response team needs to know how to detect, isolate, and recover from agent-level compromise. If they don’t, your mean time to recovery will stretch into hours or days rather than minutes.
4. Who owns the AI agent when something goes wrong?
This is the governance question that makes executives uncomfortable. Because the answer is rarely clear.
Is it the product team that deployed the agent? The data science team that trained the model? The IT team that provisioned the infrastructure? The vendor who supplied the platform? The CISO who signed off on the risk assessment?
The accountability gap is structural. Most organisations have distributed AI ownership across multiple functions without defining consolidated accountability when things go wrong. That ambiguity is a governance failure — and it will surface the moment a material incident occurs.
Fortune and Yale’s Center for Ethical Leadership and Innovation published an analysis this week warning that the latest generation of agentic AI models has exposed a fundamental gap in corporate governance. Most boards and C-suites lack the structures to oversee AI systems that autonomously execute consequential decisions.
This is not an AI safety issue in the technical sense. It’s a board-level governance and liability question. And every technology executive is accountable for it.
5. Are we retrofitting governance or building it into the pipeline?
AI governance frameworks must be embedded in the delivery pipeline — design, build, deploy — rather than bolted on at audit or review time.
If your organisation treats AI governance as a compliance checklist that gets reviewed after deployment, you’re building technical debt into your AI estate. And that debt compounds with every new agent you release.
The World Economic Forum’s January 2026 research positioned AI governance not as a compliance cost but as a growth strategy differentiator. Organisations that build governance-first agentic AI programs will have compounding trust and resilience advantages as regulations tighten. Those that don’t will face escalating costs, reputational damage, and regulatory penalties.
The CISO-CIO partnership that boards need to demand
AI governance cannot sit solely with the Chief Data Officer or the data science team. Enterprise risk requires cross-functional ownership.
CIOs need to co-own AI governance with the CISO. This is not optional. The technical delivery organisation owns the deployment pipeline. The security organisation owns the risk posture. Neither can govern agentic AI effectively in isolation.
The most mature organisations are building joint accountability structures: combined governance committees, shared risk registers, co-owned incident response playbooks, unified board reporting.
If your CISO and CIO are not co-presenting AI governance posture to the board, you have a structural gap.
What this means for you
If you’re a CISO, you need an agentic AI security posture before the deployment wave arrives. Retrofitting governance after agents are in production is significantly harder and more expensive.
If you’re a CIO, you need to co-own the accountability framework with your CISO. You can’t delegate AI governance to the data team and expect the board to accept that division of responsibility when an incident occurs.
If you’re a board member, you need to be asking evidence-based questions about autonomous AI deployment. Board AI literacy is now a governance gap risk. Boards need AI advisory capacity — and they need to be asking the accountability questions that executives would prefer to defer.
The EU AI Act enters full enforcement in August 2026. US state legislation is advancing rapidly — Colorado has enacted comprehensive AI governance requirements; California and New York are advancing similar frameworks.
The regulatory environment is tightening. The threat taxonomy is formalised. The governance frameworks exist.
The only question is whether your organisation will build accountability structures proactively — or reactively, after the first material incident.

The question is whether the CISO had the governance structures, the budget authority, and the organisational accountability to prevent it.
Most don’t. Yet.
